I am trying to help my girlfriend find her fundamental key note and I am having some trouble. She had her name changed when she was 18. Should we use the name on her birth certificate or her new name? Would the same apply for those who are married and take the name of their spouse?

The birth name is karmic. Begin with that. Yet, the new name also has an influence, so you have a lot of calculating to do.
