So by self observing, comprehension in the moment comes naturally?

It can, for superficial things. But to really comprehend, one must meditate.

How do we finally make progress beyond this point to make much more profound change?

Meditation.

Your physical perceptions only see the most superficial and illusory appearances. To see the reality, one must take the consciousness out of the body, affections, and the mind. This is only possible by mastering meditation.

And before you feel like that is impossible, it is not.

Self-observation is the first step. We have to see the ego in action. But learning not to act upon the desires of the ego is the work with the transformation of impressions.

Transformation occurs once we’ve not only seen the ego in action, but renounced its desires through comprehension in the moment. This is how we learn to follow the voice of conscience, inner judgment.

However, to reach profound comprehension, one must meditate. There is no other way.

When we find ourselves in a situation where something inside gets stirred up, whether that be anger, jealousy, envy etc and it produces the effects; physical, raised blood pressure, heart rate goes up and so on then the emotional response and mental, we notice these, not necessarily where they precisely originated but we note the effects, then we realise that we are in an altered state whichever that may be, or rather which combinations they may be, and we stop ourselves from reacting to the outside world from these states however difficult it is in order to work towards something good as we choose not to let our evil spill outside into the third dimension.
