Because a detailed description of the causes of this illness are not described in the book, an answer would help me help others. If there is already a lecture or web page that answers these questions, please send which ones.

In the 2008 publishing on page 30 Samael Aun Weor writes "A serious damage in the Mental Body, when reflectively transmitted in the physical brain, produces madness. A disconnection between the Astral Body and the Mental Body causes furious madness."
The two questions are: What causes a serious damage in the Mental Body? What causes a disconnection between the Astral Body and the Mental Body?

The astral and mental bodies are the repositories of the mind, thus they are damaged by psychological impacts, not physical ones.
Damage to the mental body can be caused by abusing the intellect (ie. Nietzsche), obsessive thinking or analysis, through fanaticism (fascism, religious fanatics, political fanatics, etc), addictive behaviors (any fixated, obsessive behaviors, like watching too much tv, constant internet use, video games, etc), and many, many other ways. It is damaged by alcohol, drugs, sexual fantasy, sex addiction, and masturbation. Certain black mystical or spiritual techniques (done in this life or the previous one) damage the mental body.
Oftentimes we do not want to recognize that we have done something wrong, so we do not "think" about it: this separates intellect from emotion. We "compartmentalize." This principle illustrates how when a person has engaged in witchcraft, black tantra, killing, raping, abusing others, etc., these severe actions can "split" the astral (emotion) and mental (intellect). The person becomes very divided internally. Again, this has symptoms of "multiple personalities," etc. Of course, this is also facilitated by alcohol, drugs, misuse of sexual energy, etc.

Astral or mental acts like cutting do not harm the bodies, since the laws are different. However, they can be harmed, and badly. For instance, black magic, taking drugs, alcohol, performing degenerated acts (especially sexually) creates enormous damage to our astral and mental aspects.

That got me thinking then, it possible to harm the astral or mental bodies in the astral plane when undergoing accidents that resemble physical injuries? i.e. getting hit by an astral car or cutting oneself?
It makes sense that beings in hell don't die immediately from the burning and cutting and so forth because their destruction comes from the gradual pulverization of the psyche.
Anyone with ego is a hasnamuss (of the first type). Since we have many egos, we have multiple personalities.
However, "multiple personalities" that are easily visible to others (ie. through the personality) is an indication of deep fractures in personality and the internal bodies: a sign that the psyche is devolving.
Worse still is when someone has created some or all of the soul, yet has not dissolved the ego: that is a fully-fledged hasnamuss, and a very dangerous creature.
